You may receive a W9 form to complete if you are hired to perform services for another company but are not an employee. If the company is required to file a return, it must obtain your Taxpayer Identification Number in order to file. People who are asked to fill out W9 forms include freelancers, consultants and independent contractors.

Instructions

      Fill out your name and business name, and then enter the type of business it is.

      Include your address, city, state and ZIP code.

      Enter your taxpayer identification number. For sole proprietors, this is usually the owner's Social Security number. For other types of businesses and some sole proprietors, it is a federally assigned identification number called a Taxpayer Identification Number (TIN).

      Sign the form in part II if you are required to according to the instructions on the form at the IRS website (see Resources below).

      Give the form back to the person or company requesting your information. Do not send it back to the IRS.

      Keep copies of the W9 forms you send out. At tax time, you can then compare your list with the 1099s you receive. If you are missing any 1099s, you will know who to contact right away.

      Use a fill-in .pdf form at the IRS website if you have many W9 forms to complete. Keep a copy with your information on it. Send copies to customers or employers who request them.
