Wipe down your equipment. No one likes to sit down in a puddle of someone else's sweat.
Step2
Rerack your weights. Some people lift a huge amount and then leave the heavy plates on the machines. This is not good for the next person that comes along to use the equipment and can't lift the weights you left behind.
Step3
Don't hog equipment. If there are a lot of people staring at you and asking if you are almost done, you probably should speed up your workout or move on to another machine.
Step4
If you are provided towel service, don't leave them lying around the locker room, put them in the hamper when you are done.
Step5
Ask if someone is done with a machine if it looks like they might still be using it.
Step6
Don't hog a large section of the locker room by spreading all your stuff around.

Don't stare at women in the gym, it makes them uncomfortable. Unless you're hot, then you can pretty much do whatever you want.
In the guy's locker room it's not polite to stand closely to other members in the room, when you're walking around with no towel and everything swinging around after your shower, especially when the other guy is sitting down, and especially if you put one leg up on the bench next to them while you try to talk to them about your workout.
