1
Draw a pattern onto a piece of cardstock with pencil. Cut it out, being careful of all the small details. This will be used a stencil. If any corrections need to be made, do them on the cardstock in stead of trying to make correction on the tile.
-
2
Trace onto the tile from the stencil. Use a wax resist, found at craft stores or use a simple Sharpie marker. A Sharpie works wonders because it is not only easy to use control, but will disappear when the tile is fired.

3
Paint on the tile with glazes. These glazes can be found at most crafts stores. Don't be fooled by the color they appear to be primarily, many change after being fired. The glazes can be applied using the tip of the bottle, and they can be spread evenly and pushed into small areas with a fine paintbrush.

4
Fire the tile in a kiln. This will set the glaze into the tile permanently. It will need to be fired at least 1000 degrees. If you do not have access to a kiln, many towns have pottery ceramics workshops that may allow the work to be done there, and may even provide the materials.

Recruit the help of custom tile makers if this is too creative of a task. There are many artists and companies out there that would love your business. Some even have website, where a photo or piece of artwork can be emailed and the finished product mailed.
