How to Obtain a Disabled Parking Permit in West Virginia

If you are a disabled resident of West Virginia, you can file an application for disabled parking placards or for disability plates with the West Virginia Division of Motor Vehicles. Your disability must meet the definition of qualified disability as detailed in West Virginia law. Your physician must certify that your disability is a qualified disability per the DMV rules for issuing disability plates and placards. Read on to learn more.

Instructions
  1. Step 1

    Print the disability parking application form DMV 41-TR from the West Virginia DMV website.

  2. Step 2

    Review the qualifying conditions listed to determine if you qualify. They include mobility limitations, respiratory issues and cardiac conditions.

  3. Step 3

    Complete Section 1 which is the applicant information. Select whether you are requesting the disability plates or the disabled parking placards. You must also certify that you have a mobility impairment which limits your ability to walk.

  4. Step 4

    Give the application to your physician to complete Section 3 of the form. Your physician must certify that you meet one or more of the conditions listed and whether the disability is temporary or permanent. You should verify that all parts are completed to avoid delay in processing.

  5. Step 5

    Take the completed application form to the nearest West Virginia DMV Regional office for processing. Alternatively, you can mail the application to the DMV address shown on the form. There is no fee for temporary or permanent disability placards. If you are applying for disability plates, enclose the applicable fees that are shown in the instructions.
