Permanent Disability Plates and Placards
Step1
Ascertain that you qualify as disabled under New Jersey MVC regulations and that the disability can be certified as qualified under the law by your physician.
Step2
Call the New Jersey MVC at (888) 486-3339 to request the disability permit application Form ISM/SP-41. The form is not available on the MVC website.
Step3
Complete all of the applicant sections on the front after receiving the application form.
Step4
Give the application form to your physician to complete the certification on the back.
Step5
Mail the application to the New Jersey MVC at the address on the form. The permanent disability plates and/or placards and handicapped ID card are mailed to you after the application is approved.
Temporary Disability Placard
Step1
Determine if you qualify as disabled under New Jersey Motor Vehicle Commission regulations and that the disability can be certified as qualified under the law by your physician.
Step2
Go to your local police department to get an application for a temporary disability placard.
Step3
Complete the applicant portions of the application form.
Step4
Take the application to your doctor to certify the temporary disability warrants a placard.
Step5
Submit a completed application form and pay the temporary placard fee at the police department to receive the temporary placard and the ID card.
