Step1
Review the conditions that qualify for a disabled parking permit on the Rhode Island DMV website. Determine if your disability meets any of the criteria.
Step2
Download and print the application for disability parking privileges from the Rhode Island DMV website. Complete the front page of the application which includes an authorization for your physician to release your medical records to the DMV so that they can adequately assess your application.
Step3
Ask your physician to complete the medical information on page 2 of the application form. Verify that your doctor includes all the requested information because an incomplete application will not be processed by the DMV.
Step4
Send the completed application to the Rhode Island DMV at the address shown on the form. After receiving the application, the DMV may request additional medical documentation which will extend processing time.
