How to Obtain a Disabled Parking Permit in Rhode Island

Disabled residents can apply for a Rhode Island disability parking permit. The disability must meet one or more of the criteria defined in Rhode Island Division of Motor Vehicle (DMV) rules and regulations, such as impaired mobility, limited ability to walk or blindness. 1. Review the conditions that qualify for a disabled parking permit on the Rhode Island DMV website. Determine if your disability meets any of the criteria.
2. Download and print the application for disability parking privileges from the Rhode Island DMV website. Complete the front page of the application which includes an authorization for your physician to release your medical records to the DMV so that they can adequately assess your application.
3. Ask your physician to complete the medical information on page 2 of the application form. Verify that your doctor includes all the requested information because an incomplete application will not be processed by the DMV.
4. Send the completed application to the Rhode Island DMV at the address shown on the form. After receiving the application, the DMV may request additional medical documentation which will extend processing time.

• Information received from your physician can affect the status of your driver's license resulting in restrictions or suspension. If there is a restriction placed on your driver license, it will not affect the approval and issuance of the disability parking permit.
