Log on to Connecticut's official state government website.

Click on "Quick Links" and "State Agencies." Select "Go."

Scroll down to "Motor Vehicles."

Click on "Special Plates" and "Choose a Special Plate." Select a category to view. Choose from animals, cities and towns, colleges, environment, military, organizations, police and fire, recreation, varied or request to view all of them.

Read over the specialty plate descriptions, eligibility requirements and the sponsoring organization's contact information, if provided.

Download and print Form MV-22.

Print the name and address of the person to whom the vehicle is registered in Section One. Include a daytime telephone number.

Write the vehicle's information in Section Two. Report the vehicle's present plate number, the date on which the registration expires, its make, year, model and vehicle identification number. Indicate whether or not the vehicle is leased.

Choose an address where you would like your specialty plates to be mailed. Write it down in Section Three.

Personalize your specialty license plates. Be sure to follow all the rules outlined on the form regarding personalized message choices. Provide six alphanumeric characters combinations in order of preference. Refer to your specialty license plate's category and description to determine how many characters are allowed on a plate.

Select your specialty plate style. Note the additional fees next to the specialty license plate titles.

Sign and date your application.

Step 13
Total your fees. Make a check payable to the DMV.

Step 14
Mail your application to DMV, Special Plate Unit, 60 State Street, Wethersfield, Connecticut, 06161-0504.

Receive your specialty license plates and use in place of your standard plates.
