Step1
Identify the website you prefer to use to create your online registration form. If you prefer to use external resources, consider websites that charge small or no fees for usage.

Determine the layout of your registration form and the type of information you require from your registrant. Most forms begin with personal contact information such as name, company, address, telephone number and email address. Subsequent questions depend on the type of information pertinent to your event. Such information may include special accommodations, flight preferences, dietary needs, accessibility, and participation for scheduled activities.

Select specific formatting that is appropriate for the question. For open-ended questions such as name or company, provide text fields to fill out the information. For answer-specific questions, provide a drop-down menu to limit answer input. An example would be specific dates for hotel accommodations.
Step4
Include open-ended fields for providing additional information, adding comments or asking questions.
Step5
Test your completed registration form. Look for errors or possible improvements to the form. If possible, request that a friend or colleague complete the form and provide comments or suggestions.
