Submit supporting documents for your application. This includes original or legible copies of transcripts for all degrees you earned, front and back copies of any other teaching licenses you hold and official NTE or GRE test scores if you have them.
-
4
Place your materials in the following order: Form A goes first, followed by your transcripts, certificates from other states, Form V, test scores if you have them and finally Form E. Do not fold down corners, staple or clip your materials together, as this will delay the processing of your application through the automated system.

5
Write a check payable to Department of Public Instruction and place on top of the materials you assembled in the previous step. Personal checks, certified checks and money orders are acceptable forms of payment for the processing fee.
