How to Obtain a Disabled Parking Permit in Missouri
Persons who are disabled in Missouri can apply for a disabled parking space permit. A person with a permanent disability can request disabled license plates or a disabled parking placard. A person with a temporary disability can only request a disabled parking placard. In either case the applicant must meet the definition of disability in accordance with Missouri laws. Review the definition of physically disabled per state of Missouri laws and determine that you qualify as a disabled person in accordance with those laws.

Request the "Application for Disabled Person Plates/Placard Form DOR-2769" from the Missouri Department of Revenue (DOR) website. This form cannot be downloaded from the DOR site.

Complete the applicable sections, once you receive the form, as they pertain to your disability.

Download and print the "Physician's Statement for Disabled License Plates/Placard Form 1776" from the Missouri DOR website. Give the form to your physician so that the type of disability can be certified and that it is either a permanent or a temporary disability. The expected length of a temporary disability must also be shown.

Submit the forms to the Missouri Motor Vehicle Bureau. If applying for a temporary disability placard, which is not to exceed 180 days, there is a nominal fee. There is no fee for a permanent disability placard or for disability plates.

A temporary disability placard can be renewed for up to another 180 days. The renewal requires that another application form DOR-2769 and medical statement Form 1776 be submitted.
