      Download and print the "Application for Persons with Disabilities Parking Permit Form 411055" from the Iowa DOT website. Disabled license plates can only be issued for permanent disability. Windshield placards can be issued to persons with temporary or permanent disability. An alternative to disability plates are a sticker which is placed on the lower right hand corner of the rear plate.

      Provide a statement from a physician or other qualified medical professional on their stationery stating that the applicant has a disability and indicating if the disability is either temporary or permanent.
