How to Get a Disabled License Plate in Michigan

A permanent disability is tough on an individual, but it's even tougher on a driver. Ease your mobility worries by applying for a permanent disability license plate. Have one less worry on the road with accessible, accommodating and guaranteed parking.

Instructions
  1. Step 1

    Log on to Michigan.gov. Click on "Secretary of State," "Owning a Vehicle," "License Plate Store," "Disability Plates/Placards" and "Disability Plates."

  2. Step 2

    Download an application for a disability license plate.

  3. Step 3

    Complete part one of the application. Provide the vehicle owner's name, their driver's license number, address, daytime telephone number, county, the permanently disabled person's name and driver's license number.

  4. Step 4

    Enter the vehicle's information. Include its year, make, body style and vehicle identification number.

  5. Step 5

    Indicate if you are a disabled veteran, own a van or use a wheelchair.

  6. Step 6

    Provide your current plate number and its expiration date.

  7. Step 7

    Sign and date your application.

  8. Step 8

    Take your application to your physician and have him detail the specifics of your disability on the form. Have him certify the truthfulness of the application by signing and dating it, providing his address and licensing information.

  9. Step 9

    Make a copy of the completed application for your records.

  10. Step 10

    Mail your application to Michigan Department of State, Distributed Services Unit, P.O. Box 30764, Lansing, MI 48909-8264.

  11. Step 11

    Receive your permanent disability plates in 2 weeks.

Tips & Warnings
  • Permanently disabled plates remain valid for up to 4 years. After 4 years, you will have to have them renewed.
