Step 1
Pick a style. It doesn't matter how many rustic, old, antique items you put into your living room, it's not going to look right if they don't somehow fit together. You need to select a theme you're going for. Country, western, cowboy, farm, southwestern or Native American are all rustic theme choices. It can pretty much be anything you choose, as long as it's old. Once you pick the style, you need to remain consistent with it.

Step 2
Change out your carpet. Analyze what type of floor covering would really be appropriate with your theme. Maybe it's hardwood. Maybe it's tile with a southwestern rug. It's entirely up to you, but consider your style when picking out new flooring. If you have relatively plain flooring (or are on a budget where new flooring is just not an option), you can make it fit better by adding an area rug in the appropriate theme. Go to your local flooring store to get advice!

Step 3
Change the walls. This will make a noticeable difference when you go to decorating your living room. You can either paint them in a color or method that would be appropriate for your style, or visit the local wallpaper store for wallpaper that would be ideal for your new rustic living room. If you've got standard off-white walls and don't want to mess with painting, that's fine. But, remember to dress up your room with appropriate framed prints and artwork.

Select the right furniture. If you have a Victorian couch, it's probably not going to go with your new cowboy themed living room. Stay consistent with your theme. Select couches, chairs and tables that are either fairly basic and can go with lots of styles or furniture that fits your new decor perfectly. If you can't afford new furniture, purchase slip covers that would be more appropriate. Add a splash of your theme with blankets, throws or pillows that make the room seem to come together more.

Step 5
Add personality. This is the time to bring out all the old stuff. It doesn't have to be in perfect condition. If it was, it wouldn't be "rustic." Just make sure your collection of rustic accessories fits together and adheres to your theme. Don't forget to include your walls. That fabulous throw could be hung from the wall. Baskets can hang from the ceiling or on the wall. Be creative with your artifacts. Make your rustic living room a statement of your own personality.
