Instructions

1. Logon to Illinois' "Cyber Drive Illinois" website at cyberdriveillinois.com.

2. Find a personalized license plate application by following the links: "Vehicles," "Departments," "Vehicle Services," "License Plates" and "Personalized License Plates."

3. Download form # VSD414.14.

4. Read the instructions on the first and second page of the personalized license plate form.

5. Cut the application portion of the form along the dotted line.

6. Print your personal information, such as your name, address and daytime telephone number.

7. Provide your vehicle's information, including the current license plate number.

8. Write three choices for your personalized license plates in order of preference. Use acceptable alphanumeric character combinations, as per the rules and regulations outlined on the form.

9. Indicate if you would like the next available plate in a series if your personalized license plate choices are unavailable.

10. Select your plate type.

11. Mail the completed form to Secretary of State, Vehicle Services Department, 501 S. Second St., Rm. 524, Springfield, Illinois 62756.

12. Receive another application in the mail from the Secretary of State and complete within 45 days.

Tips & Warnings

• You may attach more than three choices for your personalized license plates on a separate sheet of paper.

• Do not send money with your initial form. Wait until the Secretary of State sends you an official application to pay fees.

• Requests for indecent, obscene or misleading personalized license plates will be denied.
