How to Change the Name on a Passport
You may require a name change on your U.S. passport for any number of reasons. Follow these steps to alter your passport.

Use a passport expediting service if you need your new passport immediately. Such services often can get the passport to you within 1-3 days.

Obtain your passport renewal safely by visiting a government acceptance agency (such as your local post office).

Have your current passport on hand to turn in in exchange for your new one.

Bring several photographs of yourself. Photographs must meet official standards, which means they must be approximately 2" x 2" with a plain white background. Photos must be recent.

Complete the Passport Application Form (Form DS-82), which can be obtained online.

Provide legal name change documentation. Acceptable forms include a marriage license, professional decree, or professional name change documentation.

Provide Proof of Departure documentation for rush or emergency processing. Acceptable documents include an international airline or cruise ticket, or a letter from an employer verifying your iminent international business travel.

Provide all necessary documents to the appropriate agency to complete the transaction.

Passport renewal can be a lengthy process. Apply as soon as possible before your scheduled departure.
Beginning September 2005, the U.S. Passport Services Office will no longer change valid U.S. passports due to a legal change of name. Instead, you must apply for a replacement.
