      Open Microsoft Access 2003. Either select the "Access" icon on your desktop or find it by selecting "Start" (bottom left-side of your computer), then finding it in Programs.

      Press "F11" once you are in Access 2003 to switch to the database window.

      Select the "Forms" button, under objects on the left side of the screen. Double-click on "Create Form Using Wizard".

      Choose the fields you want to view on your form, in the next screen that appears. Click "Next".

      Choose the layout you want for your form. Click "Next".

      Select the style you want for your form. Click "Next" again.

      Name your form. Now, click on "Open the Form and Enter Information".

      Click "Finish".

Tips & Warnings

  • If you plan on printing your form, use a lighter background under Style Options.

  • To customize your form, click view on the menu at the top of the screen, and then "Design View".

  • Include only relevant fields in your form or it will become cluttered.
