Things You'll Need:

Step 1
Use the F11 key to open the Database window. Click on the Forms tab.

Click New. A dialog box appears.

Select the wizard that you want to use. In this example, we will use the Form Wizard.

Click OK.

Select the tables and fields that you want to include on your form. Click Next.

Step 6
Select how you want your data to be viewed. Click Next.

Step 7
Choose your layout. Click Next.

Step 8
Select your style. Click Next.

Step 9
Name your form. Click Finish. Your form is now ready to be used.

on 3/1/2007 I am Adewale, studying Information Technology and International business. I have a work on creating a microsoft access database on drug history of patients in a hospital. I am strugling with the tables to use because all the tables seems to not working on entity relationships.
Here is tips:
I need patients information table, patient drug history, patient provisional records, Doctor's information table, and Doctor's medical clerking table.
Please can you give me a guideline on the primary keys to use for each of these tables so as form an entity relationship between them.
