How to Create a Form on a Web Page With FrontPage
Forms are useful for collecting information, registering visitors and soliciting feedback on your Web page. Use the Microsoft FrontPage FormPage Wizard to make form creation simple and quick. This tutorial uses FrontPage 2000.

Start FrontPage and open an existing web.

Choose New from the File menu and select FormPage Wizard in the dialog box. Read the next dialog box and click Next.

Click Add to add a question or other form field. Choose from the provided options and edit the prompt. Click Next.

Select the User Input options for this field following the directions listed. Click Next when finished.

Add another question. Use the Move Up/Move Down commands to change the order of the questions; use Modify to change a question and Remove to delete a question.

Choose how the questions will be presented in the form.

Select whether you'd like a table of contents for your form. This usually isn't necessary unless the form is very long or the user needs to answer certain questions.

Check the box for tables if you'd like the Wizard to align the form in a table. This is extremely useful for making a form page look like a real form. Click Next when finished.

Choose the Output Option that best fits your needs. The Save Results on a Web Page option will create a new Web page with the responses and store it on your server; the Text File option will create a text file that's saved on your hard drive; the Custom CGI Script will send the results to a CGI script you designate.

Type the Base Name of the results file. This will tell you where the results are stored. Click Next.

Click Finish when all questions are complete. The form page will be generated.

Make any desired changes and save the page when finished.

Preview the page in a Web browser by choosing Preview in Browser from the File menu. From the dialog box, choose which browser to use and what settings.

Tips & Warnings
Add the form to an existing Web page using the copy and paste feature. Select the form and choose Copy from the Edit menu. Open the existing page, place the cursor where the form will appear, and choose Paste.
In FrontPage 2000, you can add individual form components by choosing Form from the Insert menu and choosing the desired component.
Not all browsers support forms or tables, so be sure to offer an alternative.
