How to Make a Form in PageMaker
Adobe PageMaker is a document-creation tool that includes all of the necessary functionality for you to create forms for readers to complete. PageMaker was the predecessor of the Adobe InDesign application and features many of the basic tools upon which InDesign has since expanded. Using the tools in PageMaker, such as the Text tool and the Selection tool, you can generate a form that anyone opening the file can complete.

Click "File" at the top of the window and then click "New."

Specify a size for your document and click "OK."

Click "Layout" at the top of the window, and then click "Column Guides."

Type "2" into the "Number of Columns" field. Click "OK."

Click the "Text" tool from the toolbox at the left side of the window.

Click at the top of the page and type a name for your form.

Click at the top of the left column and type a label for the first form field that readers of the document will need to fill in. For example, you could type "First Name" if readers will be providing that information.

Click the "Pointer" tool in the toolbox at the left side of the window.

Click the text that you just typed. Click the bottom-right corner of the text box and then drag the box so that it is completely inside the left column.

Click the "Text" tool from the toolbox again and click in the right column.

Press the "Space bar" on your keyboard three times. This will define the input box for the form field so that PageMaker will identify it as a page element.

Click the "Select" tool in the toolbox again.

Click the text box in the right column, click the bottom-right corner of the text box and drag the text box so that it is contained within the right column.

Continue adding form fields and form field labels until your form is complete.

Click "File" at the top of the window, and then click "Save."

Type a name for the document and click the "Save" button.

Tips & Warnings
You can also use the Constrained Line tool in the toolbox to draw horizontal lines underneath each empty form field in your document. This can provide additional visual structure to the form and reduce any confusion that may arise.
