How to Get a Duplicate W-2 From Your Workplace
When filing a tax return with the United States government, it is necessary to have a completed W-2 Wage and Tax Statement form. This is the form that employers use to inform both you and the government of the amount of money that you've made over the last calendar year, and the amount of taxes that were withheld. The law requires employers to send this to you no later than February 14. If the form becomes lost in the mail or you do not receive it for any other reason, you will need to get a duplicate form from your employer.

Check your workplace's employee manual. In some cases employers may outline the exact procedure that employees must follow in the event that a W-2 is lost and a duplicate is needed.

Visit the human resources department at your place of employment. In most workplaces this is the department that sends out your W-2. If your employer does not have a human resources department, then speak with your manager about who to contact. Explain that you need a duplicate of your W-2 and why.

Provide your employer with your current mailing information. This is crucial, particularly if you are requesting a duplicate because you never received the original.

If you no longer work for an employer whose W-2 form you need to file your taxes properly, then visit the former workplace if possible. If not, then call, email, or send them a letter requesting a copy of your W-2 statement. They are still required by law to provide you with this information.

Contact the Internal Revenue Service if you do not receive the requested copy of your W-2. You will need to provide the IRS with information such as your employer's name, address, and phone number, as well as your name, phone number, social security number, and the dates of employment. The IRS will contact your employer for you with regard to your W-2.

Tips & Warnings
Do not postpone sending in your tax return, even if you have not yet received your duplicate form. When you call the IRS, they will also send you Form 4852, Substitute for Form W-2. Complete this and turn it in with your taxes.
