A woman is not legally required to change her last name when she marries. A wife may choose to hyphenate or use only her maiden name. It is important to understand that whether you choose to hyphenate or assume your husband’s surname, it does not happen automatically. Certain steps are necessary for a wife to legally replace her maiden name with her husband’s last name.

  • Driver's license or picture ID card

      Notify your place of employment of your intent to change your name before the ceremony so that they can begin the process of changing your name on email addresses, business cards and other work-related documents.

      Complete the “Application for a Social Security Card” form SS-5. The form can be downloaded from the Social Security Administration website.

      Take the completed form to your local Social Security office along with a certified copy of the marriage license, your Social Security card and a valid picture identification card. The Social Security Administration will issue you a temporary Social Security card with your new last name.

      Go to your local department of motor vehicles office to change the name on your driver’s license or picture ID card. You will need to bring your temporary Social Security card, a certified copy of the marriage license and your current driver’s license or picture ID card with you.

      Contact banks, vehicle registration departments and any other companies you do business with to notify them of the name change. You may be required to complete additional forms and submit certified copies of the marriage certificate and your new Social Security card.

      Verify with your employment’s human resources department that your new name, as well as your married status, are reflected on your tax deductions and income withholdings.
