1
Machine wash and dry your cotton fabric. Iron the fabric as soon as it is done drying so the wrinkles don't have time to set. Pre-washing the fabric keeps the pillow from warping from shrinkage during its first wash.
-
2
Fold the fabric in quarters. Cut the fabric using pinking shears in a half-heart shape that starts and ends on the single folded edge. There is an edge made of two folded edges from the first fold you made; don't cut on this edge. Make the half-heart take up as much of the fabric as possible while still maintaining a pleasing shape. You now have two identical fabric hearts.

3
Cut 3-inch-deep slits every inch around the perimeter of the hearts after putting them together with the right sides facing out. Tie together the corresponding strips created by the cuts. Leave one of the straight edges of the heart untied.

4
Take a basketball-size chunk of fiberfill. Pull the fiberfill apart several times in several different directions bringing it back together between each pull. This prevents the pillow from becoming lumpy. Stuff the chunk of fiberfill in the heart-shaped case. Continue pulling apart chunks of fiberfill, and stuffing them in the case until the pillow is firm.

5
Tie the strips together along that last straight edge to finish closing the pillow case around the filling.
