How to Complete an IRS 1096

Internal Revenue Service Form 1096 is used to transmit paper forms to the IRS. It tells the IRS the number of forms included when you are filing your federal income taxes through the mail. You submit one Form 1096 for each type of paper form you file. If you have 250 or more of any one type of form, you must file them electronically. After you complete each payee's tax form, complete Form 1096 and include it with the forms when submitting them to the IRS.

      Separate each form by type. For example, separate your 1099-Rs from your 1099-SAs.

      Count each form type and document the number.

      Obtain one Form 1096 from the IRS website for each type of form you are filing. For example, obtain one Form 1096 for all of your 1099-Rs and one form for all of your 1099-SAs.

      Enter the filer's name, address, phone number and employer identification number or Social Security number in the top section of each form. This information must match that on the other forms you are filing.

      Enter the total number of forms included in the return in the box labeled "Total Number of Forms."

      Check the box at the bottom of the form to indicate which type of form you are filing. For example, if you are filing Form 1099-R, check the box labeled "1099-R."

      Enter the total amount of federal income tax withheld shown on the forms you are submitting with a particular Form 1096 in the box labeled "Federal Income Tax Withheld."

      Enter the total amount of the payments on the forms you are submitting with a particular Form 1096 in the line labeled "Total Amount Reported With This Form 1096."

      Check the box in the "Final Return" section if you won't be required to file Forms 1098, 1099, 3921, 3922, 5487, 8935 or W-2G in the future.

      Sign your name, enter your title and date the form. Mail each information form along with the corresponding Form 1096 to the IRS address listed on the bottom of Form 1096.

Tips & Warnings

  • If you are filing Forms 1097, 1098, 1099, 3921, 3922, or W-2G, you must file by Feb. 28.

  • If you are filing Form 5498, you must file by May 31.

  • If you have more than 250 of one form, refer to IRS Publication 1220 for information on filing electronically.
