How to Get Shoal Shells in "Ruby"
In “Pokemon Ruby,” the Shell Bell is an item that gradually restores your Pokemon’s health during battle. While this item is useful, you won’t find it for sale in PokeMarts; instead, you’ll need to collect the ingredients for the item from Shoal Cave in eastern Hoenn. The item requires that you collect four Shoal Salts and four Shoal Shells -- the latter of which you can only obtain during high tide.

Travel to Mossdeep City, an island east of the Hoenn mainland. If you’ve previously visited this location, you can return there with the Fly ability.

Surf north from Mossdeep City until you reach Route 125. Wild Pokemon, such as Tentacool, Wingull and Pelipper inhabit this area. Spray repel to ward off these wild Pokemon. You can also expect to meet several rival trainers who specialize in Water-type Pokemon.

Enter Shoal Cave on the west side of Route 125. When you enter the cave, the area must be experiencing high tide, which takes place from 9 a.m. to 3 p.m. and 9 p.m. to 3 a.m.

Travel northwest through the cave until you find a doorway in the rear of the first chamber. Enter the doorway.

Collect the four Shoal Shells throughout the upper chamber. You will find one in the northwest corner of the room, one in the center of the room and two along the eastern wall.

Tips & Warnings
Return to the cave during low tide to collect Shoal Salts. Once you have all of the ingredients for a Shell Bell, talk to the old man in the first chamber of the Shoal Cave. He will make a Shell Bell for you.
