      Complete Form 4506-T to request a copy of your tax return. You need to provide your name, address, Social Security number and birth date. Also note the tax form, such as Form 1040, you want and the year or years for which you request a transcript. If you want the transcript sent to a third party, you must include that address as well.

      Look up the address to which you should mail your Form 4506-T on the second page of the form. The location to which you mail the form depends on where you lived at the time you filed the return. For example, if you want a transcript of your 2009 tax return and you filed in Illinois, but you now live in Kentucky, use your former Illinois address to determine where to send the Form 4506-T.
