How to Fill Out a 1040-ES

You must use IRS Form 1040-ES to make quarterly estimated tax deposits when you do not have enough withheld to cover your federal income taxes due for the year. Self-employed individuals must also make quarterly deposits on Form 1040-ES. The IRS requires estimated tax deposits when the tax balance from your last tax return is more than $1,000.

      Determine the amount of your quarterly 1040-ES payment. The simplest way to calculate your quarterly payment is to look at the balance you owed on your last tax return. This is shown on the line labeled "Amount You Owe." Divide the amount you owed by four. The result is the quarterly estimated tax deposit you must make on Form 1040-ES. Alternatively, you may use the 1040-ES worksheets to calculate your quarterly estimated tax deposit due.

      Obtain Form 1040-ES from the IRS website. The form contains four vouchers to make your quarterly payments.

      Select the 1040-ES voucher form that corresponds to the quarter you make the payment in. In general, the first quarter payment is due April 15, the second quarter payment is due June 15, the third quarter payment is due September 15 and the fourth quarter payment is due January 15.

      Write the amount of your deposit in the upper-right corner boxes. Include a check or money order for that amount.

      Write your name, Social Security number and address in the appropriate boxes.

      Locate your state in the "Where to File" section of the 1040-ES instructions. This section begins on page three of the Form 1040-ES booklet. Remove the voucher you complete from the booklet, attach your payment and mail to the IRS address shown for your state.
