How to Make a Contact Sheet With CS5

Adobe Dreamweaver CS5 lets you create a contact Web page, so users can send you questions and feedback from your website. Dreamweaver CS5 has a wizard to help you create the contact form, so you do not need to manually type any HTML code. The contact sheet is set with your email address, so the contact data is sent directly to your email inbox.

      Click the Windows "Start" button, and click "All Programs." Click "Adobe," then click "Dreamweaver CS5" to open the software. Open your Adobe Web project after the software loads.

      Click the "Insert" menu item, then click "New." Choose "HTML" to create a new HTML page for the contact form. Dreamweaver creates a new page with all the default HTML code.

      Click the "Insert" menu item again, and select "Form." A form inserts on the Web page. A properties panel displays at the bottom of the window. This is where you set all of your form properties.

      Type your email address in the "Action" text box. The "Action" is the page or email address to which the form information is sent.

      Click "Insert," then "Form," then "Text." This options adds a text box to the form input. Continue this step for each piece of information you want to collect from the user. For instance, add a text box for the reader's name, email or phone number and a feedback text box.

      Click "Insert," and "Form," then "Button." Select "Submit" as the button type. This is the button used to send the feedback to your email.

      Click "Save" to save the new Web page and contact form. Open the page in your Web browser to review and test the new form.
