How to Embed a Form Into Facebook
With the more limited options that came during Facebook's overhaul in 2011, you may find it harder to place items such as videos, pictures and third-party content on your Facebook page or profile. Although no simple way exists to embed a form into Facebook, if you really want to display a form that may be used by potential clients, you can employ a work-around to accomplish the goal.

Open the form within the graphics or word-processing program where it was created.

Click "File," from the top menu, then "Save As."

Select "JPG," "GIF" or another type of image file from the "Save File As" box that appears on your screen. Depending on the type of operating system you have, this selection may be displayed under the section where you select the files or folders to save the file to. The important thing here is to get your file saved as an image file, which will display with a .jpg or .gif file extension once it's saved.

Click "Save" to save the file to your computer.

Log in to Facebook with your username and password. If you want the form saved on a Facebook page instead of your personal profile, navigate to that page now.

Click "Photo" then "Upload a Photo From Your Drive." Select the file you saved in Step 4 to begin uploading the file to Facebook. When the file is loaded, click "Publish" to publish the form to your Facebook albums. The form will now appear among your photos. If you want to direct potential clients or fans to use the form, instruct them to click "Download" under the image within Facebook, then have them print the form on their computer. The form will not be interactive -- meaning users can fill it out within the platform -- but they will have access to it.

Tips & Warnings
If your form is more than one page or you're having other issues with posting your form as an image, the other option is to host the form on your website and to place a link in your Contact Information page, giving people a place to always find a link to the form.
