How Can I Get a Copy of My 2005 W-2 Form in Texas if I Lost It?
A W-2 is a wage and tax statement used in the United States to report wages paid, as well as income taxes paid. All employers are required to complete a W-2 for each employee they pay a salary. Employees use the W-2 to file their individual income taxes. If a W-2 was never received or lost after receiving it, individuals can obtain another copy.

1
Contact your Texas employer or former employer who issued the W-2 and ask for a copy of your 2005 W-2 form. You may need to speak to the company's account payable department or the company's accountant.
-
2
Contact the IRS and ask for a Form 4506-T to be mailed to you. A Form 4506-T gives you access to information from your W-2 for any previous year filed at no cost to you. It is not an actual copy of your original W-2 form but will give you the same information found on the W-2.

3
Go to the IRS website and print the Form 4506-T yourself if you have access to a printer.
-
4
Complete the form and mail it to the IRS.

Print out a Form 4506 from the IRS website if you need an exact copy of your W-2. Fill it out and mail it in to receive your W-2 form from 2005 or other previous years.

6
Include a check for $57 made out to the US Treasury to receive a copy of the W-2 requested.
