      Fill out the state of Alaska voter registration application form. Print a copy of the form located online at the State of Alaska Division of Elections website or pick one up at a local Division of Elections location.

      Complete the form in its entirety. Write clearly using only blue or black ink. Include all required information, including name, signature, complete mailing address, personal contact information, social security number and driver's license information. Specify your party affiliation and disability needs, such as requiring assistance during voting, if applicable.

      Provide a copy of valid personal identification. Acceptable forms of identification include a current Alaska driver's license, U.S. passport, Alaska state identification card or a copy of your birth certificate. You must provide proof of Alaska residency with your application in order to complete the registration process. Photocopies of identification documents are acceptable if mailing or faxing the application. Show a copy of discharge paperwork related to any felonies, if applicable.
