If the rebate comes from the manufacturer the form should be included in the product packaging, but if it comes from the retailer you need to be sure and get the form from store employees.
Carefully read the form, paying special attention to what the manufacturer requires that you send.
Once you have read the form, fill out any blanks left for the consumer.
Don't forget to double-check the form. Leaving an item blank can cause you to not receive your rebate.
Also pay close attention to the time limits, as some companies give you as little as one week to submit your rebate.
Receipt and UPC Barcode

Nearly all rebates require two things be sent, a receipt and the UPC barcode from the product packaging.
Read your form carefully, as some companies require the original receipt, while others require a copy. Many will request that you circle the item, but some will stipulate that the receipt be free of writing.
Regardless of what they request retain a copy for yourself. -
Follow Up

If for some reason you do not receive your rebate in the time specified, contact the company that issued it.
Remember, most companies that offer rebates do it in the hopes that the rebate will either not be redeemed, or the process will be too complicated to be redeemed properly. This is the time when you may need copies of your receipt or rebate form.
If there is a problem, patience and diligence will normally win you the rebate that is owed to you.
