How do I Purchase a New Louisiana Birth Certificate?

You can purchase a new Louisiana birth certificate very easily.

You can obtain copies of your Louisiana birth certificate fairly easily and for only a small fee. Perhaps you lost your original certificate, or you might need a certified copy for legal reasons. Either way, you can request a copy employing several different methods.

Application

You can obtain an application from the Louisiana Vital Records Registry website (see Resources). The form is fairly brief, but you must fill it out entirely. You will be asked for personal information concerning the person listed on the certificate, as well as information about yourself if you are applying for someone else. You also must submit a copy of a photo ID.

Fees

There is a $15 fee for a standard long-form birth certificate or a $9 fee for a short-form certificate. You can pay by check, money order or major credit card. Make checks out to "Louisiana Vital Records Registry." You will be charged an additional fee for using a credit card because the Vital Records Registry Office uses a third-party business to process credit card payments.

You can deliver your application in person, by mail, via Internet or fax. The address for the Vital Records Registry is P.O. Box 60630, New Orleans, LA 70160. The telephone number to pay the fees (which requires a major credit card) is 877-605-8562.
