The car accident report is assigned a district control number for identification. The accident location will be indicated, and the approximate time of the loss is recorded.

Contributing factors may help determine fault for the accident. Examples include, but are not limited to, traffic control devices, negligent actions, and weather conditions.

The car accident report will record the points of impact and the severity of damage for each vehicle. Damage is identified as minor, moderate, or heavy.
