Runway Safety

The Federal Aviation Administration is concerned about runway safety, in which pilots may land on the wrong runway, obstruct other pilots or make errors while taxiing.
Collisions

Collisions with other aircraft, equipment or birds are not uncommon, such as the 2009 U.S. Airways New York flight that collided with a flock of geese but resulted in no fatalities.

Impaired Pilots

Law enforcement has implemented tough measures to ground pilots under the influence of substances. Once example occurred in November 2009, when a United Airlines pilot ready to depart from London was arrested for failing an alcohol breath test.
Passenger Misconduct

"Air rage," in which a passenger's disruptive behavior threatens aircraft safety, increased aboard British planes from 2,702 in 2007-08 to 3,529 in 2008-09, according to the BBC.
Maintenance Issues

Negligent maintenance is a serious safety issue. In 2003, improper repairs on a U.S. Airways Express resulted in two crewmembers killed in a crash.
