Spironolactone, also known as Aldactone, is a synthetic steroid. Most commonly used for treating high blood pressure, the drug is sometimes also used for controlling hormonal acne.   1. Mechanism of Action

    • Excess androgen hormones can trigger acne in females. Spironolactone belongs to a category of drugs called anti-androgens that prevent cells from absorbing these hormones. By reducing circulating levels of androgens, acne is brought under control.

    • For acne, spironolactone is usually given in a daily dose of 50 g to 100 g. The dosage may vary depending on the condition of acne in a patient.

    Duration

    • Duration of treatment depends on the individual patient and the severity of her acne; it can be used for a week to a year or longer. Spironolactone is considered safe for long-term use, according to the result of a study.

    Specific Usage

    • Spironolactone is prescribed for a week only before the period, on a monthly basis, to women whose acne is limited to the premenstrual phase.

    Spironolactone and Oral Contraceptives

    • Prescribing oral contraceptives with spironolactone is common practice for treating acne. The drugs complement each other in calming the hormones responsible for acne.

    Side Effects

    • Menstrual irregularities and breast tenderness are the most common side effects associated with the drug. Patients with a history of kidney disease, breast and ovarian cancers are advised to stay away from spironolactone.
