Serious complications with intrauterine devices (IUDs) like Mirena are rare but may occur. These includes pelvic inflammatory disease (PID), ruptures of the uterine wall, ovarian cysts and irregular bleeding. Additionally, if pregnancy occurs while the IUD in place, serious complications may ensue.

Pelvic Inflammatory Disease

According to the Mayo Clinic, recent IUD insertion is a risk factor for developing PID, since bacteria can enter the uterus during insertion and infect the reproductive organs. If left untreated, this may lead to infertility.
Uterine Perforation

According to Mirena's prescribing information, perforation of the uterine wall may occur while the device is inserted. If left untreated, the perforation may allow the IUD to move around and harm surrounding organs.

Ovarian Cysts

Safety information from Mirena's manufacturer, Bayer Healthcare, indicates use of an IUD may increase the risk of ovarian cysts developing. Ovarian cysts may present few symptoms, however, if they rupture IUD users may experience severe pain and shock.
Irregular Bleeding

According to prescribing information, changes in bleeding patterns are the most common adverse reaction cited during clinical trials. These patterns may remain irregular and menstruation may stop entirely, a condition known as amenorrhea.
Pregancy Complications

There is also a minimal risk of becoming pregnant while using Mirena. There is also an increased risk to IUD users for ectopic pregnancy, serious pelvic infection, early labor and miscarriage.
