What Documents Do I Need to Get a Passport?

Any U.S. citizen who wants to travel from and back to the United States must first obtain a passport. This passport proves the holder is a U.S. citizen. It also allows residents to get visas to visit other countries.

    Form DS-11

  1. Form DS-11 requires you to provide basic information about yourself, some of which will be included on the passport. Do not sign this document until directed to do so at the passport interview.
  2. Evidence of U.S. Citizenship

  3. Your birth certificate is often the easiest form to submit to prove citizenship. Immigrants can show naturalization certificates.
  4. Identification

  5. Most people use driver's licenses to prove identification. Government-issued ID or a military ID can also be used.
  6. Identification Photocopy

  7. In addition to showing your ID, you must also provide a photocopy of it. The photocopy should be on an 8.5-by-11-inch piece of paper that shows the front and back of the ID.
  8. Photos

  9. You must submit two identical color photos of your face that are 2 by 2 inches.
