The Change of Name Act
The Change of Name Act is Canadian legislation detailing the stipulations and regulations of legally changing your name in Canada. The act is generally straightforward but varies slightly depending on the province/territory in which you seek the name change. The reasons for seeking a name-change can vary from simple dislike of your current name to a desire to disassociate yourself from family.

Eligibility

To change your name in Canada you must be the legal age of majority (18 or 19 depending on province) and be a legal resident of the province, either through birth or by having lived there for a period of time that satisfies the Canadian government. Residents under the age of majority can still change their names provided they have the consent of their legal guardians, have been married or are in a common-law relationship.

Proof of identity (in the form of a birth certificate, immigration papers or permanent resident card) is required to apply under the Change of Name Act. Change of Name Act applications also include extensive background checks of your work history, marital status, possible criminal background, previous name changes and residency history.

Submission and Fees

Depending on the province/territory, you must submit the application to the Ministry of Government Services, the Canadian Court of Justice, the Civil Status Registrar or the local Office of Vital Statistics. Application fees vary from $10 in the Northwest Territories to $185 on Prince Edward Island.
Justification

Applications submitted under the Change of Name Act must include a justification for the name change. Applicants must explain why they wish to have their names changed and if the government considers the reason valid it will grant the change.
Disqualification

Canadian name change applications may be disapproved if the applicant has frequently changed names in the past or is trying to change to a name which may reasonably cause identity confusion. Additionally, people who are trying to change their names for the purposes of avoiding financial obligations or criminal charges are disqualified.
